Belize Agricultural Health Authority (BAHA). It was an easy process. Call the number below and they will tell you what you need to fax them for the permit (info about your dog...breed, age, sex, etc...)

Requirements:
Domestic pets will be allowed to enter the country provided that owners present the following:

valid import permit
international veterinary certificate (needed 7 days prior to departure)
valid rabies vaccination certificate
inspection by quarantine officer
US $30 entry fee + US $12.50 fax fee

Approved permits will be faxed to applicant at a cost of US $12.50 to be paid at the point of entry on the day of arrival.

Persons who forego the application process will be subject to a US $100.00 violation fine in addition to the US $30 entry fee.

I just do not know the requirments/documents to take your pet back as Karmin and I are still here.

You will have to check with the US the requirments for bringing your pet back. I assume you will need to see the vet here for an international certificate to go back. It's like your pet's passport.

It should be easy taking your dog Houston-Belize City, such an easy trip. I had to go Las Vegas-Houston-Belize.

I recommend taking the water ferry from BZE-SPR.

You need a health certificate from a vet in Belize to transport your dog back to the US. It's only valid for one week so you'll have to schedule accordingly. The vet is only on the island on Wednesday's and you can call the clinic at 226-3266 for an appointment.

Timing of your vacation is also an issue, there are embargoes enforced by the airlines and if you need to make sure that you don't come down while it's open and try to go up when it's closed.
