For those who have been to Caye Caulker already: Would you advise for or against taking antimalarial medications at all? I get different answers depending on who I ask.

You might want to take anti-malarials if you are going inland for a few days. Otherwise, I wouldn't bother.

If you are going to do that, but will be here for several weeks, you might consider buying the meds there, where they will be cheaper and will not require a doctor's appointment. Then after two weeks you could head inland.
