You mentioned hiking boots. How difficult a climb is it and how long is the hike? One of the people on our trip has recently broken his foot. It is healed, but wouldn't want to put him in jeopardy of maybe re-injuring it. Have you heard anything about the Nekton Pilot? Thanks

By Far, the Caves Branch River trip is the best in Belize. It's offered currently by two tour groups that I know of.
Ian Andersons Jungle lodge does two different float trips in two different caves. I think Caves Branch is best( the other is St. Hermans Cave). Ian offers a trip through all three cave systems on the Caves Branch (of the Sibun River).
Jaguar Paw Resort also offers a single cave float day trip in Caves Branch, but there's a bit more hiking involved.

The best way(Ian Anderson) is to Start at the first cave and take a short hike to the second system. Then they'll pick you up without too much walking after floating through the second and third. Be prepared to stay a night though.

Also, check out the Barton Creek Cave. It's a great canoe trip up the Cave with very little walking.

FYI I wouldn't recommend the Casa Maya trips. I've heard too many complaints about them.

David's Tours is great for Barton Creek trips.

Ian Anderson's is tops for a full spectrum of caving and adventure trips.
