I am Canadian and going to Belize for 5 months. I understand I need to get a 90 day visa and then report to an immigration office in San Pedro once a month. Is this correct? Also, I am near Toronto...Do I go to Embassy of Belize for application?
Any help would be greatly appreciated and approximate cost

There is no 90 day visa, you get 30 days renewable indefinitely.

Thats not true. Its not indefinitely. After 6 months they will start asking for proof of income, and can and have revoked any further visas if you cant comply. They may not do this to everyone but they do it to a lot.

Yes, I agree, you have to renew every 30 days........but I thought that you could do that for 12 months.........correct me if I'm wrong.

That won't work always: I once went to Tikal twice in a row and by re-entering Belize the second time, the immigration officer gave me a 2 week visa, stating "I was traveling up and down too much". No discussion.
You have to go to immigration every month. First 6 months it costs 50 BZD after that 100 BZD every time. Theoretically you can do that indefinitely, but the longer you stay without applying for PR or work permit, the more questions they will ask.

If you're travelling with a Canadian passport, you don't need to apply for a visa before leaving Canada or entering Belize.

When you land at the airport in Belize, your first stop is Immigration where your passport will be stamped (half-page size) allowing a maximum stay of 30 days -- no fee. Note that it's 30 days and not one month. If coming from Mexico, it will be a similar procedure.

On the last working day before the 30 days is up, go to the nearest Immigration office and ask for a new stamp (another half-page in your passport). You might be asked to show your return ticket and proof of funds (I never was, but you will have to provide your address in Belize) so be prepared for that possibility.

The fee for the new stamp is BZ$50 per stamp for the first 6 months and then BZ$100 per stamp (unless the fees have changed recently).

An Immigration office is located in each of the six Districts of Belize, and there's one on Ambergris Caye.

You won't need to leave Belize in order to get a new stamp, but you might want to make a trip to Mexico or Guatemala during your stay. In that case, I'd time those trips to begin when one of the monthly stamps is about to expire. When you return to Belize, you might get a new stamp for free (but you might not).

The border by Guat is dicey for new visas. That is where Debbie was hassled.
There is a sizable gringo community there who skips the visa thing by traveling in Guat and expecting a new visa after each trip.
The Mex border is a better choice.
You may not like San Pedro Immigration. If you go there they will not let you go to the city after that to renew after thirty days.
We always got new visas in the city.
