That's actually my point. I'd like to see an enforced legal requirement, as exists in many places, for full details of the incident to be disclosed in a formal manner, and routinely published with whatever censorship is necessary to protect people's reasonable rights - and no more. Then when these incidents occur, and there have been several others of late that have never received any publicity, we won't be left guessing and wondering if there should be a lesson there that we could all learn from.

I agree with Elbert that the operator is usually blameless and deserves our sympathy, but on occasion it seems that the operator could by doing things differently have avoided the problem altogether. I'm not talking about this incident of course, as like most people I know little about it.
