We will provide the following passport services on a first-come, first-served basis. Please bring the items listed.

Adult U.S. passport renewal
� your current U.S. passport
� completed form DS-82
� 2"x2" passport photo
� one clear photocopy of your current passport biographic data page
� BZ$220 (bank draft ONLY )

Child Under 16 U.S. passport renewal - Note: BOTH parents must be present if listed on the birth certificate.

� your child's current U.S. passport
� completed form DS-11
� 2"x2" passport photo
� one clear photocopy of your child's passport biographic data page
� your child's original birth certificate and one photocopy
� each parent's ID and one photocopy of each
� BZ$230 (bank draft ONLY ) (Note: Fee for first adult passport renewal (age 16 and over) is BZ$290)

CASH WILL NOT BE ACCEPTED. All payments must be made by bank draft.
Please visit a local bank where you have an account to purchase your bank draft. If you do not have a local bank account, visit Atlantic Bank and indicate the draft is for payment of U.S. Embassy consular fees. The draft should be payable to "U.S. Embassy" for the amount indicated above. You will need to present acceptable ID, provide your Belize address, and pay a fee of BZ$16.10 to the bank to purchase the draft.

All passports will be returned via DHL/SpeedIt. The cost is BZ$33.75 payable in cash on delivery.

Lunch will be available for purchase.

I wonder if DS-11 can be accepted in plain paper only, or it can be filled out without printing from the DOS website? Or this one https://ds11.pdffiller.com/ at the very least?

I recently retired as a passport acceptance agent for the Postal Service in Idaho. Unfortunately the State Dept. does not accept electronic forms. It has to be a hard copy. The DS-11 has to be signed in front of and witnessed by an acceptance agent. Before I retired I was told by the Passport Agency that they have had hopes of rolling out an electronic DS-82 (adult passport renewal) but had been having some difficulties.
